ADMISSION REQUIREMENTS
Whether arriving from a hospital, or coming directly from home, we require
that the following records be received:
Medical History & Physical Examination Report: This information must be
current, within five days (Medical Facilities), thirty days (Assisted
Living, Apartment/Suite Neighborhood) prior to admissions and must be
completed by a physician.
Tuberculin Skin Test or a Montoux Test: This must be done within three
months prior to admission or on the day of admission. This will be done by
our facility if not done previously.
Admission or Transfer Orders: These must be signed by a physician and
include current medication (prescriptions and over the counter), special
diet, and other special orders for care. A letter to your physician listing
necessary orders will be provided if you are coming from home.
Pre-Admission Screen Program (PASAR): This needs to be completed and
approved by your local Area Agency on Aging for admissions into the
Barnesville health Care Center. We can assist you with contacting the local
office at 740-425-3648. If coming from a hospital, most discharge planners
will handle this screening for you.
Personal Physician: We encourage your personal physician to monitor your
progress at our facility. If your physician is unable to visit, our medical
director will be glad to work with your physician to provide with the best
care possible. You may retain your own physician if that physician will
follow your care at BHCC.
Admission Paperwork: We want to make the day of admissions easy for
everyone. Once you arrive at our home, our hospitality service team will
greet you and your family and escort you to your room.
After your are comfortable we can assist in unpacking and getting you
settled in.
A family member, responsible party, or Power of Attorney needs to proceed to
the Business Office so that the appropriate paper work for admissions to our
center can be completed. The exception being if prior arrangements were made
with our Admissions/Marketing Director or Social Services Director.
To assist in the admission process, please bring the following documents, if
applicable, to be photocopied for our files:
Social Security Card
Medicare Card
Insurance Card
Medicaid Card
Guardian/Conservator Papers
Power of Attorney Papers
Living Will and/or Durable Power of Attorney for Healthcare Papers
Do Not Resuscitate (DNR) Papers
